Filing A Claim

With the DHMO, you don't have to complete and submit claim forms when you visit your PCD or an approved DHMO network specialist.

However, if you receive emergency care from a dentist other than your PCD, you may have to submit a claim form. Contact PMI Customer Relations for more information about emergency care.
