Participants Who Are Unable To Be Located
If a benefit payment under
the Fund cannot be made because the Fund's records do not include the
individual's current address or the individual does not cash the check from the
Fund by the last day of the plan year following the year the claim was incurred,
then the Fund will consider the benefit forfeited. The Fund will reinstate the
benefit of any individual who presents himself/herself to the Fund after such
forfeiture has occurred. However, such reinstatement will not include any
adjustment for increases or decreases in the benefit for the period between the
date of forfeiture and the date of reimbursement.
