Hospital Claims
Hospital Claims Submission Guidelines
Each hospital claim filed with the Health Fund should include the following information:
- Participant's Name
- Participants Health Fund ID Number (as it appears on the Health Fund ID Card)
- Hospital's Name
- Hospital's Address
- Hospital's Federal Tax ID Number
- Patient's Name
- Patient's Date of Birth
- Procedure Code (provided by hospital on bill)
- Diagnosis Code (provided by hospital on bill)
- Amount Paid (if any)
You should make sure that the bill given to you by the hospital is in the UB-92 format. If the form is not in the UB-92 format, it may delay the processing of your claim. See below for hospital claims submission addresses
