Benefit Plan Checklist
	Go over the rules concerning the limited disability extensions to coverage and COBRA continuation coverage rules. Continued Health coverage is an essential ingredient to dealing with your disability.
	Should you decide to apply for the disability extension of coverage, please complete the Disability Application Form and have the Attending Physicians Statement completed by your doctor and return them to the Administrative Office along with any supporting documentation. You will be notified of the decision in writing.
	If your disability is a Terminal Illness, review the Terminal Illness section of the Pension BenefitTabs™ to review the benefits available.
	You should check your vesting status under the Pension Plan to determine whether you are vested for a pension benefit. If you are not vested with the Pension Plan, your disability may affect your Qualified Years towards vesting, depending on the duration of your disability.
