Same Sex Domestic Partnership
The Health Fund and the Pension Plan recognize same sex domestic partnerships as long as certain requirements are met. By satisfying the requirements, your same sex domestic partner will generally be treated as a spouse. Please see the Benefit Plan Checklist for details.

You may add your same sex domestic partner as a Covered Dependent under the Health Fund and/or a Qualified Domestic Partner for the Pension Plan. If you would like to add your same sex domestic partner as a Covered Dependent under the Health Fund or for Pension Plan purposes, please contact the Health Fund Eligibility Department to request a Same Sex Domestic Partner ("SSDP") packet. You will be required to file an affidavit of domestic partnership which is included in the packet, and submit along with it three of the supporting documents named below. In addition, you will also be required to pay a quarterly tax on the imputed value of the benefits your same sex domestic partner will receive. There is also a quarterly dependent premium payment required by the Health Fund. If you would like to add your same sex domestic partner as a Qualified Domestic Partner for Pension Plan purposes, you may designate on the Affidavit of Domestic Partnership that you would like the Affidavit to apply to the Pension Plan.

The Affidavit of Domestic Partnership for the Health Fund and/or the Pension Plan requires supporting documents, which include, but are not limited to:
domestic partnership agreement
joint mortgage or lease
designation of domestic partners as each other's life insurance or retirement plan beneficiaries
designation of domestic partners as each other's primary beneficiary in will
joint ownership of motor vehicle or of primary checking account or primary joint credit account
other documentation, satisfactory to the Health Fund or Pension Plan, confirming mutual financial support.

In addition, if you reside in a jurisdiction that issues a certificate of domestic partnership, you must obtain such a certificate and furnish a copy to the Administrative Office. The State of California issues such certifications. This certification is not required for the Health Fund, but may be used as one of the supporting documents required.

The Affidavit of Domestic Partnership for the Health Fund and the Pension Plan are one and the same. By filing the Affidavit of Domestic Partnership with the Administrative Office, you may designate your same sex domestic partner as your Qualified Domestic Partner for the Pension Plan only, the Health Fund only, or both

You may want to review your beneficiary designation with the Health Fund and the Pension Plan and make any changes you deem appropriate. If you do not fill out a new beneficiary designation with either the Health Fund or Pension Plan, any life insurance that may be due will be paid to the beneficiary listed on the most recent designation and any Pre-Retirement Death Benefit will be paid according to applicable Pension Plan rules. If you would like to change your beneficiary designation for your life insurance, you must complete a Health Fund Life Insurance Beneficiary Card. You should complete a new Designation of Beneficiary Form for the Pension Plan, either to name your Qualified Domestic Partner as your beneficiary or have your Qualified Domestic Partner agree to the named beneficiary. Keep in mind that since a Qualified Domestic Partner is now on file with the Pension Plan, your Qualified Domestic Partner's notarized signature is required on the Designation of Beneficiary Form if you and your Qualified Domestic Partner decide to designate a beneficiary other than your Qualified Domestic Partner as the sole primary beneficiary. To change your beneficiary designation for the Pre-Retirement Death Benefit under the Pension Plan, please complete a Pension Plan Designation of Beneficiary Form. Remember that changing your beneficiary for the Health Fund does not change your beneficiary for the Pension Plan and vice versa. Please see Forms referenced on this page.
